Safety Investigation Training
​This course teaches delegates how to plan and explore a safety occurrence effectively, from beginning to end. Collecting and exploring evidence, creating a timeline, and identifying both barriers which have failed, as well as exploring which processes were successful. Human Factors will be covered as well as the variety of techniques, methods, and tools available to identify organisational causality. The theory will run alongside a simulation, to ensure delegates can practice at every opportunity, culminating in effective language use, recommendation development and peer to peer feedback.

What does the course involve?

This 4-day course comprises a series of taught seminars, group discussions and participatory activities.

Includes the 2-day Human Factors within Safety Investigations Course.

Explores how to apply Human Factors within Safety Investigations, including effective use of language, understanding performance variability and adaptive capacity, investigation models, and questioning techniques.

Brings together the entire Safety Investigation Process, including analysis of data, interviewing techniques, barrier identification, organisational causality, and recommendation development.

Alongside taught elements, you will get to practice various elements of the process through participation within an investigation simulation.
